Discharge Medicines Service

The Discharge Medicines Service (DMS) is an essential service provided by community pharmacies to support patients discharged from hospital with medication changes made in hospital.  The service aims to improve outcomes, prevent harm, and reduce readmissions.

Patients are referred to their local community pharmacy on discharge by the discharging hospital so that the pharmacy can help patients with medicines reconciliation and optimisation.  Pharmacies will liaise with hospitals and GP practices where disparities are identified.

Currently the interface between Lincolnshire’s hospitals and pharmacies has prevented any patients being referred in to DMS.  However, from 15th November 2021 patients discharged from NWAFT may be referred into DMS, so practices in South Lincolnshire may start receiving queries from pharmacies about patients’ medications post-discharge. 


We will inform practices as this rolls out to other providers.
