Care Coordinator– Frailty Team (30 hours/week)
Help deliver proactive, person-centred care for our frail patients as part of a supportive and forward-thinking PCN team.
Salary: Up to £25,000 (pro rata, dependent on experience)
Role Overview
We are looking for a compassionate and organised Care Coordinator to join the Frailty Team at Trent Primary Care Network (PCN). This is a new and rewarding opportunity to make a real impact by supporting some of our most vulnerable patients.
Working across both community and PCN settings, you will play a vital role in coordinating care, supporting patients in navigating services, and working closely with our multidisciplinary team to provide personalised, holistic support.

Salary: Up to £25,000 (pro rata, dependent on experience)

Hours: 30 hours per week

Contract: 18-month fixed term
Key Responsibilities
-
First Point of Contact: Be a trusted link for frail patients and their families seeking support.

Appointment Coordination: Arrange assessments, follow-ups, and health reviews.

Accurate Record Keeping: Maintain clear, up-to-date patient records in line with PCN protocols.

Service Navigation: Help patients access the right health, social, and voluntary sector services.

MDT Engagement: Attend and contribute to multidisciplinary team (MDT) meetings and care planning sessions.
Person Specification
Essential:
-
Experience working in a health or social care setting
-
Excellent organisational and communication skills
-
Confidence in using IT systems and maintaining records

A patient-focused, empathetic approach

Ability to work independently and collaboratively
Desirable:
-
NVQ Level 3 in Health and Social Care (or equivalent)

Knowledge of frailty, long-term conditions, and complex patient needs
