Advanced Nurse Practitioner
Full-time 37.5 hours (5 days) – 30 days leave plus B/holidays.
Start date: around the 1st of Jan 2024.
Monday to Friday – Rare Saturdays depending on the Clinical need
About Us
Kirton Medical centre is a relatively small practice which is constantly growing, serving a list of 8,000 patients. We have a diverse multi-disciplinary team with 3 GP partners, 2 ANP`s, 3 Nurses, as well as other allied colleagues such as First Contact Physiotherapists and Pharmacy colleagues. We are also very well aligned with our Boston PCN colleagues.
We are a Dispensing practice in the heart of the village. We pride ourselves on nurturing and supporting professional development and look forward to welcoming a new team member into our growing KMC family.
We have an excellent family like workforce to which we wish to add an Advanced Nurse Practitioner to support our Same-Day clinical appointment system. Appointments will be a mix of phone calls, e-consult and face to face, as well as care navigating to other services and supporting and empowering the reception team to do this too.
Roles and Responsibilities
The following are the core responsibilities of the ANP. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
- Pre-triage e-consult online consultations and same day access
- Consulting with patients either F2F, by phone or by e-consult
- Supporting the duty doctor with medical complexity
- Provide home visits when needed.
- Supporting reception with care navigation
- Good communication with the team and patient
- Good use of IT systems- S1/e-consult/AccuRx/Intranet
- Special interests welcome and will be nurtured e.g. Diabetes, Women’s health, Paediatrics, Minor Surgery.
- Support mentoring and supervision of colleagues/other health professionals as appropriate within skill set.
- Increased Focus on Multi-Disciplinary Clinical education
- Have a sense of humour.
