Hours: Full Time – 37.5 hours/week
Contract: 18-Month Fixed Term
Reports to: PCN Clinical Director / Age Matters Team Lead

About the Role
Trent Care PCN is seeking an experienced and motivated Occupational Therapist to join our Age Matters Team. This role is part of a proactive, multidisciplinary approach to frailty and preventative care, working across a variety of community and clinical settings.

You will support patients aged 16+ with frailty or functional difficulties, offering holistic assessments, short-term rehabilitation, and care coordination in line with the DES PCN Contract and Lincolnshire’s Five-Year Older People’s Strategy. Your work will help prevent hospital admissions and promote safe, independent living.

Key Responsibilities

  • Conduct person-centred OT assessments and functional reviews

  • Use recognised frailty identification tools to guide early intervention

  • Carry out home visits, assess environmental risks, and support rehabilitation

  • Work collaboratively within the MDT, including GPs, physiotherapists, social prescribers, and community partners

  • Contribute to care planning, discharge coordination, and health promotion

  • Maintain clinical records in line with professional standards

  • Deliver training and service development activities within the PCN

Essential Criteria

  • Degree/Diploma in Occupational Therapy

  • HCPC registration

  • Experience working with older adults or those with frailty

  • Strong assessment, care planning, and communication skills

  • Community working experience

  • Ability to work independently and within a team

  • Full driving licence and ability to travel across the PCN area

Desirable Criteria

  • Knowledge of the DES PCN Contract and Lincolnshire’s strategic priorities

  • Experience in integrated or primary care teams

  • Training in dementia care, falls prevention, or digital health solutions
