Main duties of the job
The successful applicant will be able to support our lovely nursing team to care for our patient population.
Main duties will include baby and adult immunisations and vaccinations, wound care, smears and long term condition annual reviews.

Job description
- be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within scope of practice to the entitled patient population.
- support delivery of clinical areas such as health promotion, chronic disease management, health prevention
Primary Responsibilities:
- Support nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Manage vaccination and travel clinic service
- Manage dressing clinics to provide wound care (ulcer /Doppler etc.) to patients
- Request pathology services as necessary
- chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required, and meeting qof targets

- Baby immunisation clinic
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively
- Maintain chronic disease registers
- Implement vaccination programmes for adults and children
- Prioritise health issues and intervene appropriately
- Recognise, assess, and refer patients presenting with mental health needs
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
- Support the clinical team with all safeguarding matters, in accordance with local and national policies
- Deliver opportunistic health promotion where appropriate
- Maintain baby immunisation results logs

- Ensure compliance with ICP regulations

Experience
Essential
- Experience of working with a nursing team.
Desirable
- Experience of working in General Practice preferred.
- Long term condition management, including respiratory conditions
- Good IT skills – we are currently an Emis Web Practice but will be changing to System One in February.
