We are looking for an experienced Practice Nurse to join our friendly team.
You must hold First Level Registration, have a broad clinical experience, be able to work autonomously, have excellent interpersonal skills and a flexible approach to the delivery of care.
20 hours per week, salary is dependant on experience.

Job Description
Job Title: Practice Nurse
Responsible To: Practice Manager
Based at: The Brant Road Surgery & The Springcliffe Surgery
Liaise with: All Staff employed by the Partnership and associated staff.
Main Purpose of the Job:
- To provide patients with a high standard of nursing care.
- Accountable for the assessment, planning, implementation and evaluation of the patients care.
- Undertake such clinical nursing procedures as are determined appropriate to the post.
- To provide healthcare sensitive to the needs of the local population.
- To provide support and supervision for junior staff and students.
Essential / Desirable Skill / Experience appropriate to this Post:
- First Level Nursing Qualification and current NMC membership.
- Marie Curie Cancer Care Certificate for Cervical screening or equivalent.
- Evidence of educational training in Immunisation and Vaccinations.
- Evidence of educational training in chronic disease management (e.g. diabetes, asthma, COPD).
- Evidence of continuing professional development.
- Evidence of basic computer skills.
- Effective communication and interpersonal skills.
- Commitment to embark on future training / education as required by the service.
The Nursing Service:
- Work within the national, local and practice guidelines.
- Work as a member of the multidisciplinary team.
- Demonstrate excellent time management skills to manage workload.
- Manage resources efficiently resulting in cost effectiveness and assisting the practice to meet financial obligations.
- Contribute to identifying community health needs and strategies to address them.
- Provide evidence-based care to a variety of patients.
- Contribute to the health promotion strategy within the practice.
Clinical Practice:
- Provide patients with a variety of nursing services to promote healing and maintain health.
- Promote an evidence-based culture through pathways and protocols of care utilising an interdisciplinary approach.
- Treat patients appropriately in line with local and national guidelines according to their diagnosis.
- To undertake Chronic disease management (e.g. diabetes, asthma, CHD, COPD, Hypertension, AF & CKD) within National and Local guidelines (e.g. NICE, BTS, PACEF), including reviewing and monitoring, progress, response to treatment and advising the clinical team on the current treatments specific to your areas of expertise.
- Interpret test results (e.g. Peak flow and Spirometry) as per current BTS/NICE/GOLD guidelines to diagnose asthma/COPD and record accordingly.
- Make appropriate referrals to members of the multidisciplinary team.
- Make professional decisions for which he / she are accountable.
- Provide patients with an evidence-based treatment, that is regularly reviewed, which incorporates, cultural preferences, health beliefs, behaviour and traditional practices.
- Relate theory to practice.
- Review medication for therapeutic effectiveness in accordance with evidence based practice and national / local guidelines (e.g. NICE, BTS, PACEF) and work with the clinical team and patients to support adherence to prescribed treatments. Particular groups of medications and/or specific medicines to be agreed by the GP Partners and post holder.
- Undertake health screening and health surveillance within a broader health promotion / public healthy strategy.
- Admit and discharge patient from the caseload.
- Screen patients for disease risk factors and early signs of illness following National Service Frameworks.
- Deliver and promote adherence to the national childhood immunisations and national vaccination programmes as directed by the Department of Health.
- To support all practice services and undertake reviews relevant to QOF, DES, and individual patient outcomes.
- Maintain accurate records.
- Assess, develop, evaluate and implement programmes of care.
- Use clinical skills and knowledge within the Scope of Professional Practice.
- Administer medicines following patient group directions and patient specific directions.
- Any other duty as required commensurate with the post for which the post holder has the necessary experience and/or training.
